3 A RESOLUTION OF THE CITY OF BOYNTON
4 BEACH, FLORIDA, APPROVING AN ADDENDUM
5 TO BROKER AGREEMENT FOR THE ONE YEAR
6 RENEW AL TO CONTRACT WITH PUBLIC RISK
7 INSURANCE AGENCY FOR INSURANCE BROKER
8 SERVICES IN THE AMOUNT OF $12,000 FOR THE
9 YEAR; AND PROVIDING AN EFFECTIVE DATE.

11 WHEREAS, on September 18, 2008, the City Commission of the City of Boynton
12 Beach awarded a Broker Agreement to the Public Risk Insurance Agency for Broker
13 Services for the Renewal of the Current Insurance Programs (RFP #065-1610-08/JA) in
14 the annual amount of$12,000.00; and I
15 WHEREAS, that Broker Agreement has been extended one time for a two year
16 period from September 18,2009 to September 17,2011; and
17 WHEREAS, staff is recommending approval of an Addendum to Broker
18 Agreement for renewal of the Annual Contract for Brokerage Services for the Renewal of
19 the current insurance programs for one additional year in the amount of $12,000.00.
20 NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION
21 OF THE CITY OF BOYNTON BEACH, FLORIDA, THAT:
22 Section 1. The foregoing "Whereas" clauses are hereby ratified and confirmed as
23 being true and correct.
24 Section 2. The City Commission of the City of Boynton Beach, Florida, hereby
25 approves the Addendum to Broker Agreement for a one year renewal of the Agreement with
26 Public Risk Insurance Agency for insurance broker services in the amount of $12,000.00 per
27 year, a copy of which is attached hereto as Exhibit "A".
28 Section 3. That this Resolution shall become effective immediately.

ADDENDUM TO BROKER AGREEMENT FOR
"ANNUAL CONTRACT FOR BROKERAGE SERVICES FOR THE RENEWAL OF
THE CURRENT INSURANCE PROGRAMS"
This Addendum shall take effect on signature by both parties.
THE CITY OF BOYNTON BEACH, FLORIDA, a municipal corporation,
hereinafter referred to as "CITY ",
and
PUBLIC RISK INSURANCE AGENCY, a Florida corporation, hereinafter
referred to as "BROKER"
WITNESSETH:
1. The Broker Agreement between the CITY and the BROKER entered into the 12th day of
November, 2008 is amended as follows:
In consideration of the mutual terms and conditions, promises, covenants and
payments hereinafter set forth, CITY and BROKER agree as follows:
Section 3. TIME FOR PERFORMANCE. Work under this contract shall commence
September 18, 2011 thru September 17, 2012.
2. All other terms and conditions of the Agreement not specifically amended shall remain in
full force and effect for the balance of the term of the agreement.
IN WITNESS OF THE FOREGOING, the parties have set their hands and seals the day
and year first written above.
